Adding Answers to Questions

Answers are added to each question on the Questions tab. After you create a question, you must add the answers that your users can select. The type of answer you can add depends on the Question type that you selected when adding the question, and whether or not you are creating a Questionnaire or Assessment.

About Questionnaires

Questionnaires can include question types that are multiple choice, select all that apply, and written response. Answers can also be marked as variant, and you can add follow-up questions answer.

  • Multiple choice questions include option type answers. You can add as many answers as you'd like, but the user can select only one answer. This type of question is useful for (but not limited to) True/False or Yes/No questions.
  • Select all that apply questions include check box type answers. You can add as many answers as you like, and the user can select more than one answer. However, when you add the question, you can limit the number of answers the user can select.
  • Written response questions provide a text box for users to type in their answers. Written responses are useful if you want the user to answer an open-ended question, provide an opinion, or provide an explanation to a previous answer. Written responses are not available in Assessments.
  • Variant answers: Answers in Questionnaires can be marked as variant. A variant answer is one that you determine is unacceptable or inappropriate for the given question. If the user selects a variant answer, the Questionnaire Reviewer can be notified. Variant answers are not available in Assessments.
  • Follow-up questions: In a Questionnaire, answers can prompt follow-up questions. Follow-up questions are useful if you want specific answers to prompt different questions, or if you want the user to provide more information or explain an answer choice. Answers to follow-up questions can also prompt additional follow-up questions. All question and answer types can be added to follow-up questions, and there is no limit to the number of follow up questions you can add. This creates a nested-list of questions and answers in your questionnaire that can get quite lengthy if overused. Follow-up questions are not available in Assessments.

About Assessments

Assessments can include question types that are multiple choice, or select all that apply.

  • Multiple choice questions include option type answers. You can add as many answers as you'd like, but the user can select only one answer. This type of question is useful for (but not limited to) True/False or Yes/No questions.
  • Select all that apply questions include check box type answers. You can add as many answers as you like, and the user can select more than one answer. However, when you add the question, you can limit the number of answers the user can select.
  • Correct or incorrect answers: In both question types, when you add the answers you must choose which answer is the correct answer. Multiple choice questions can only have one correct answer. Select all that apply questions can have more than one correct answer.
  • Feedback: You can add feedback that displays when the user reviews the completed assessment. For example, you may want to explain to a user why an answer is incorrect.
